The Polk County Sheriff's Office says 25-year-old Isaiah M. Hayes of Ashland was shot by a sheriff's deputy early Tuesday morning.  The shooting followed a police chase in Altoona, near Des Moines.

"The subject jumped out of the vehicle and had what appeared to be a weapon in his hand," Lt. Rich Blaylock with the Polk County Sheriff's Office told WOI-TV. "One of the deputies fired his duty weapon, striking the suspect."

Deputy Ryan Phillips, an 8-year veteran of the Polk County Sheriff’s Office, has been placed on paid administrative leave, which is standard procedure.

The sheriff's office said all fatal officer-involved shootings are presented to a grand jury for review.
